Office Hours
Appointment times are arranged on an individual basis and subject to availability. Initial Consultations are typically 50-60 minutes; psychotherapy sessions are 45 minutes in length.
Confidentiality
Patient confidentiality is a privilege enforced by state law & professional ethics. Treatment information will not be shared with any third party without the patient's written permission. There are particular circumstances, however, where confidentiality cannot be maintained. Examples include: if there is reason to believe the patient is at imminent risk of serious self-harm or may harm another person; if there is reason to believe that a child is being abused or neglected; if treatment information is subpoenaed by a court of law. Additionally, if a patient is using health insurance for payment, the insurance company will likely require disclosure of treatment information.
Cancellation
Patients are charged directly for appointments cancelled with less than 24 hours notice and missed sessions with no notice (i.e., "no shows").
Insurance
Dr. Leveille is an in-network provider for the following insurance companies:
· Anthem Blue Cross/Blue Shield
If you have coverage with a different insurance company that also reimburses you for psychotherapy (e.g., through an "out-of-network" provision), Dr. Leveille will provide requested material to assist with claims.
Fees
Fees and co-pays are due at the time of the appointment, unless other arrangements have been made. Dr. Leveille maintains a certain number of reduced-fee openings and such an arrangement may be possible, depending on availability and the patient's specific financial circumstances.
